Official Induction Nomination Form
The Montgomery County Chapter of the PA Sports Hall of Fame invites you to submit candidates for induction into our chapter.
Criteria for consideration are in addition to meeting the county requirement of having been born or raised in Montgomery County, nominee was a resident of Montgomery County during his/her athletic career or participated in athletics in Montgomery County.
Nomination of Living Individual
Must meet one (1) of following:
- Must have been retired from sport or position a minimum of three (3) years
- Been in the sport or position a minimum of twenty-five (25) years
- Reached their fifty-fifth (55th) birthday
Deceased Nominees
- Must have been deceased a minimum of one (1) year prior to nomination.
Please note, must furnish name of Nominee’s Representative who will represent deceased if elected.
